For decades, the standard metabolic intervention for PCOS was metformin. It worked, to a degree. But GLP-1 receptor agonists have changed the equation. And the PMOS rename, by centering insulin resistance and metabolic dysfunction, positions GLP-1 therapies as arguably the most evidence-aligned pharmacological approach to the condition.
This is not speculation. The data now includes multiple randomized controlled trials, meta-analyses, and a rapidly growing real-world evidence base. GLP-1 prescribing among women with the condition increased more than sevenfold between 2021 and 2025.
The Insulin-Androgen Link
The mechanism connecting insulin resistance to androgen excess in PMOS is direct and well-established. Excess circulating insulin stimulates ovarian theca cells to increase androgen production. Simultaneously, it suppresses hepatic production of sex hormone-binding globulin (SHBG), which is the protein that binds testosterone and keeps it inactive. Less SHBG means more free, bioactive testosterone.
This creates the characteristic PMOS hormonal profile: elevated total testosterone, elevated free testosterone, low SHBG, and the downstream effects of androgen excess including acne, hirsutism, and anovulation.
The logical implication is straightforward: if you fix the insulin resistance, the androgen dominoes start falling in the right direction.
Fix insulin resistance → reduce hyperinsulinemia → decrease ovarian androgen production → increase SHBG → lower free testosterone → improve ovulation, reduce hirsutism, clear acne. GLP-1 RAs target the top of this cascade.
What the Evidence Actually Shows
A 2025 meta-analysis of 8 randomized controlled trials comparing GLP-1 receptor agonists to metformin in women with PMOS found that GLP-1 RAs were:
- Superior to metformin for improving insulin sensitivity (measured by HOMA-IR reduction)
- Superior for reducing BMI and waist circumference
- Comparable for reducing total testosterone, free androgen index, and DHEA-S
- Comparable for improving menstrual regularity
- Comparable for reducing hirsutism scores
In other words, GLP-1 RAs do everything metformin does for the hormonal profile while being significantly better at addressing the metabolic root cause.
Individual trials add detail. Semaglutide at 2.4 mg weekly for 24 weeks produced mean reductions in total testosterone of 15 to 25 percent and improvements in menstrual regularity in the majority of participants. Free androgen index dropped by roughly 30 percent. Hirsutism scores, measured by the Ferriman-Gallwey scale, showed statistically significant improvement, though clinical improvement in hair growth takes months due to the hair growth cycle.
Semaglutide vs Metformin: Head to Head
Metformin has been the default metabolic treatment for PMOS for two decades. It is inexpensive, widely available, and has a long safety record. But its efficacy is moderate at best: typical weight loss is 2 to 3 percent of body weight, and its effects on insulin sensitivity plateau relatively quickly.
Semaglutide produces 10 to 15 percent body weight loss at therapeutic doses. But the benefits for PMOS go beyond the scale. Preclinical studies using DHEA-induced PMOS models have shown that semaglutide reduces insulin levels, lowers androgens, and decreases ovarian inflammation through mechanisms that appear independent of weight loss alone.
This is important because it means the hormonal benefits are not solely a downstream effect of losing weight. GLP-1 receptor agonists appear to have direct effects on the endocrine and inflammatory pathways involved in PMOS.
Tirzepatide: The Dual-Agonist Advantage
Tirzepatide activates both GLP-1 and GIP receptors, producing greater weight loss than semaglutide alone (up to 22.5 percent at highest doses in the SURMOUNT trials). For PMOS, the additional GIP receptor activation may offer advantages beyond weight loss, as GIP receptors are expressed in adipose tissue and may influence visceral fat metabolism directly.
There are no PMOS-specific tirzepatide trials published yet, but clinicians are increasingly prescribing it off-label for women with the condition who have not responded adequately to semaglutide or who have significant insulin resistance and obesity.
Orforglipron (Foundayo): The Pill Option
Approved by the FDA in April 2026, orforglipron (brand name Foundayo) is the first oral GLP-1 receptor agonist with no food, water, or timing restrictions. At the highest dose, it produced 12.4 percent weight loss at 72 weeks in the ATTAIN-1 trial and beat oral semaglutide head-to-head in ACHIEVE-3.

Periods, Fertility, and the Ozempic Baby Question
One of the most discussed phenomena in GLP-1 therapy is the apparent increase in fertility among women using these medications. The mechanism is straightforward in the PMOS context: by reducing insulin resistance and normalizing the hormonal environment, GLP-1 RAs can restore ovulatory cycles in previously anovulatory women.
This is clinically significant for two reasons. First, women with PMOS seeking fertility may find that GLP-1 therapy restores regular ovulation without needing fertility medications. Second, women with PMOS who are not seeking pregnancy must be aware that their contraceptive needs may change on GLP-1 therapy. Oral contraceptive absorption may also be reduced due to delayed gastric emptying.
GLP-1 receptor agonists are not currently approved for use during pregnancy. If you are on a GLP-1 and planning to conceive, discuss timing of discontinuation with your provider. The general recommendation is to stop at least two months before attempting conception.
Who Is a Candidate
GLP-1 therapy for PMOS is most appropriate for women with documented insulin resistance (elevated fasting insulin, HOMA-IR above 2.5, or metabolic syndrome features), particularly those who have not achieved adequate metabolic improvement with lifestyle changes and metformin alone. BMI above 27 with metabolic comorbidities, or BMI above 30, typically qualifies for insurance coverage for weight-related indications.
GLP-1 therapy is less likely to be the right first-line approach for lean PMOS patients whose primary dysfunction is neuroendocrine (LH/FSH-driven) rather than metabolic. For those women, kisspeptin-based approaches may be more mechanistically aligned.
Practical Considerations
Common side effects include nausea, which typically improves over the first 4 to 8 weeks with gradual dose titration. Gastrointestinal effects are the most common reason for discontinuation. Starting at the lowest dose and titrating slowly is the standard approach.
For PMOS-specific monitoring, the following labs are recommended at baseline and at 3 to 6 month intervals: fasting insulin, HOMA-IR, total testosterone, free testosterone, SHBG, HbA1c, and a lipid panel. Menstrual cycle tracking is also valuable for documenting return to ovulatory cycles.
